For the password issue, I can only think that the two switches were initially setup with different security requirements.  Perhaps a duplicate password is entered in the second unit?  Probably I would look at resetting both units back to factory default and set them up again.  Who knows otherwise if there is a subtle difference between the two units.

As for the returning the base unit back to the unit it is suppose to be on, the only way to do that is to reboot the stack.  There is no other option.

You can default the switch passwords within a stack by using:
cli password switch serial none
cli password switch telnet none
and
cli password switch read-only "userpasswd"
cli password switch read-write "securepswd"

I would think the password for one unit was weaker, then a code upgrade to SSH code was done on the stack.
