DHCP problems with 1140e and Cisco 3750 POE switch stack

I am hoping someone here can help my with this issue I have been working on for a while now.

No matter what combination of switchport configuration or DHCP scope options I have tried I still can't get DHCP to work on the voice vlan for a Nortel 1140e (F/W 0625C6E).

Here is the basic setup:

Voice vlan: 34
Data vlan: 31

Data DHCP option:
DHCP option 191 = VLAN-A:34.

Voice DHCP option:
DHCP option 128 = Nortel-i2004-A,10.1.70.5:4100,1,5;10.1.70.5:4100,1,5.

Switchport config:
interface FastEthernet3/0/32
 switchport access vlan 31
 switchport mode access
 switchport voice vlan 34
 priority-queue out
 mls qos trust dscp
 no cdp enable
 spanning-tree portfast
end

I know it is making requests to the DHCP server based on the logs, but seems like it is not tagging and making the same requests on the voice vlan:

Strange.  There were a couple replies to this post, but I don't see them any more.

I think Michael had pointed to the phone config as the possible culprit, so I will go down that path and update if I find anything.

I migrated the forums to a new server and it seems I may have mistakenly used an older backup when I restored the database, hence we lost a few post from last week. Sorry... I will try to merge them and recover them, or at least post the body of the posts.
